Job summary

Community Connection Programs, Inc. in Baton Rouge is seeking a Care Manager for Home & Community-Based Services. This full-time role, Monday–Friday starting 8:30 AM, offers $17–$21 per hour ($35,360–$43,680 annually) based on experience.

You will manage a caseload in the metro New Orleans area, with a mix of office duties and field visits. You will build relationships with clients, families, caregivers, and support coordinators, develop care plans, supervise caregivers, and ensure timely

Qualifications

Experience in healthcare coordination or supervisory roles.
Strong organization, communication, and follow-through.
Willingness to travel throughout metro New Orleans.

Responsibilities

Build relationships with clients, families, caregivers, and coordinators.
Complete home visits, quarterly reviews, and caregiver supervisory visits.
Complete monthly face-to-face visits for clients in Supported Independent Living.
Develop and update care plans, and adjust as needs change.
Keep client records, homebooks, and notes accurate and current.
Track authorization dates, required documents, visits, and deadlines.
Coordinate caregiver schedules and respond to call-outs.
Coach caregivers, address performance concerns, and communicate training needs.
Respond to concerns and incidents, document actions, and coordinate timely follow-up.
Participate in team meetings and rotating after-hours and weekend on-call schedule.
