About the Role

• Coordinating and managing client care plans, including developing and implementing personalized care plans to meet each client's unique needs
• Conducting initial and ongoing assessments to identify client needs, strengths, and goals
• Developing and maintaining relationships with clients, families, and healthcare providers to ensure seamless care coordination
• Identifying and addressing any gaps in care, ensuring continuity and quality of services
• Monitoring client progress and making adjustments to care plans as needed
