Job summary

Community Connection Programs, Inc. in New Orleans, LA, is seeking a Care Manager for a full-time role, typically Monday–Friday with standard office hours and on-site visits.

You will manage a caseload of 30+ clients, performing regular home visits, reviews, and supervision of caregivers within the metro area. You’ll develop and update care plans, keep records current, and coordinate schedules while handling on-call responsibilities as part of a rotating team.

Qualifications

• Experience in healthcare coordination, human services, home care, or a supervisory role.
• Strong organization, clear communication, and dependable follow-through.
• The ability to balance deadlines, changing schedules, and client needs.
• Experience with Louisiana Medicaid home and community-based services is preferred.

Responsibilities

• Manage a caseload of 30+ clients throughout the metro New Orleans area.
• Complete home visits, quarterly reviews, and required caregiver supervisory visits.
• Complete monthly face-to-face visits for clients receiving Supported Independent Living services.
• Develop and update care plans, and follow through when clients’ needs change.
• Keep client records, homebooks, and notes accurate, organized, and current.
• Track authorization dates, required documents, visits, and other service deadlines.
• Coordinate caregiver schedules and respond to call-outs to maintain coverage.
• Coach caregivers, address performance concerns, and communicate training needs.
• Respond to concerns and incidents, document actions, and coordinate timely follow-up.
• Participate in team meetings and a rotating after-hours and weekend on-call schedule.

Community Connection Programs, Inc. has served Southeast Louisiana since 1999. At CCP, our care managers build relationships, guide caregivers, and help keep services dependable.

What you’ll do

You’ll manage a caseload of 30+ clients throughout the metro New Orleans area. Your work combines office responsibilities with an average of 3–4 client visits each week.

• Build respectful relationships with clients, families, caregivers, and support coordinators.

• Complete home visits, quarterly reviews, and required caregiver supervisory visits.

• Complete monthly face-to-face visits for clients receiving Supported Independent Living services.

• Help develop and update care plans, and follow through when clients’ needs change.

• Keep client records, homebooks, and notes accurate, organized, and current.

What you’ll bring

Experience with Louisiana Medicaid home and community-based services is preferred. Familiarity with AxisCare, caregiver supervision, or disability services is also helpful.
