Qualifications

• Healthcare coordination or home care experience required.
• Supervisory or team leadership experience preferred.
• Strong organization and clear communication skills.
• Ability to travel throughout metro New Orleans.
• Experience with Louisiana Medicaid HCBS or related services preferred.

Responsibilities

• Manage a caseload of 30+ clients in the metro New Orleans area.
• Balance office tasks with 3-4 client visits per week.
• Build relationships with clients, families, caregivers, and coordinators.
• Complete home visits, quarterly reviews, and caregiver supervision visits.
• Develop and update care plans as needs change.
• Maintain accurate client records and notes.
• Track authorizations, documents, visits, and deadlines.
• Coordinate caregiver schedules and cover call-outs.
