• Assist prospective clients in becoming lifelong Right at Home clients.
• Supervise on-call care coordinators after hours on a rotating basis.
• Support community outreach to strengthen referral partnerships.

As our Home Care Client Care Manager, you will be the heart of our client experience. Your mission is to ensure every client receives compassionate, high-quality support and every family feels confident, informed, and cared for.

Your responsibilities will include:

• Developing, maintaining, and overseeing personalized client care plans
• Building trust and relationships with clients, families, caregivers, and referral sources
• Mentoring caregivers and supporting meaningful, safe, exceptional care delivery
• Acting as a calm, positive, solutions-focused leader- no matter how busy the day gets
• Playing an essential role in helping prospective clients become lifelong Right at Home clients
• Supervising, on a rotation, our on call care coordinators after hours
• Supporting community-facing efforts as needed to strengthen our referral partnerships
