Job summary

Hartford HealthCare is seeking a Home Care Aide to provide in-home personal care and support to clients in the Torrington area. You will follow the established plan of care and work under the direction of the client care coordinator to help clients maintain independence at home.

The position emphasizes compassionate care, reliability, and adherence to safety and care standards while coordinating with the Hartford HealthCare team and families.

Qualifications

• High school diploma or equivalent required.
• Six months' experience as a Home Care Aide or CNA in a home care setting preferred.
• Current Home Care Aide or CNA certificate approved by the CT Department of Public Health.

Responsibilities

• Provides personal care and housekeeping in clients' homes as outlined in the care plan.
• Assists with personal hygiene and grooming.
• Assists with meals and nutritional needs.
• Reports changes in client condition to supervisor.
• Performs routine housekeeping and home management duties.
• Documents care provided and maintains care plans.
• Offers emotional support to clients and families.
• Assists with routine therapy activities as directed.
• Monitors and reminds clients about medication routines.
• Participates in care conferences and required training.

• Provides personal care and housekeeping assistance to clients in their homes; follows the established plan of care under the direction of the client care coordinator.
• Provides personal hygiene and groom care to clients.
• Provides meals for clients' nutritional needs.
• Reports changes in clients' condition to client care coordinator or supervisor.
• Performs routine housekeeping and home management duties.
• Records each client's care on care plan and submits care plan in a timely manner.
• Assists in providing emotional support for the client and family.
• Assists with routine therapy exercises.
• Reminds client of medication regime, opens medicine containers for clients as needed.
• Participates in case conferences as requested.
• Attends required in-service sessions.
• Will actively participate in the performance improvement process known as H3W.

Qualifications

• Preferred Education Requirement: High school diploma or equivalent
• Preferred Experience: Six months experience as a Home Care Aide or CNA in a home care environment.
• Required Licensure: Home Care Aide or CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services
