Job summary

Hartford HealthCare at Home is seeking a compassionate Hospice Aide/CNA to join Connecticut's leading homecare network. In this role you will provide personal care, assist with daily living activities, and support families in patients' homes.

Under the direction of the client care coordinator you will follow the plan of care, document care provided, and report changes in condition. Minimum HS diploma and CT DPH CNA certification are required.

Qualifications

• High School diploma or equivalent is preferred.
• CNA certificate approved by CT DPH required.
• Six months Home Care Aide/CNA experience preferred.

Responsibilities

• Provide personal care and housekeeping in clients' homes under the care plan.
• Provide personal hygiene and grooming support.
• Prepare meals meeting clients' nutritional needs.
• Report changes in condition to the client care coordinator.
• Perform routine housekeeping and home management tasks.
• Document care in the care plan and submit promptly.
• Offer end-of-life care with sensitivity.
• Assist with routine therapy exercises.
• Participate in case conferences as requested.

CNA Job Responsibilities

• Provides personal care and housekeeping assistance to our clients in their homes; follows the established plan of care under the direction of the client care coordinator.
• Provides personal hygiene and grooming care to our clients
• Provides meals for clients' nutritional needs
• Reports changes in clients' condition to client care coordinator or supervisor
• Performs routine housekeeping and home management duties
• Records each client's care on care plan and submits care plan in a timely manner
• Responds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care
• Assists with routine therapy exercises
• Participates in case conferences as requested

Qualifications

• Education: High School diploma or equivalent, preferred
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services required
• Experience: Six months experience as Home Care Aide/CNA in a home care agency, preferred
