Job summary

Trinity Health's LIFE St Mary location in Feasterville-Trevose, PA is seeking a part-time per diem Personal Care Assistant/Home Health Aide to support elderly participants with daily living activities.

You will provide personal care, assist with mobility, oversee routines, and coordinate with nursing staff. Requires active CNA, 1 year elderly care, valid driver's license; MS Office skills and strong communication; willing to travel locally.

Qualifications

• Must be an active Certified Nurse Assistant in the state where the PACE Organization is located.
• Minimum of one year of experience working with a frail or elderly population preferred.
• Valid driver’s license required.
• Superior written/verbal communication skills.
• Customer-focused with ability to adapt to change.
• Proficiency with Microsoft Office (MS Word, Excel, PowerPoint) and basic email/calendaring.
• Position may require local travel (about 10%).
• Ability to perform physical tasks including kneeling, lifting, and walking (up to 20 pounds).

Responsibilities

• Provide personal care assistance to maintain independence in activities of daily living (toileting, bathing, grooming, meals).
• Assist with physical activity, exercise, and walking.
• Offer behavioral supervision and reminders for nursing instructions, diet, and emotional support.
• Support with conversations, recreation, meal service, and light housekeeping.

Status: Per Diem

Location: LIFE St Mary, 2500 Northgate Dr, Feasterville-Trevose, PA

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position will be housed primarily at the LIFE St Mary facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
