Job summary

Trinity Health Corp. is seeking a dedicated Certified Nursing Assistant to provide personal care and support for elderly participants at LIFE Kinder Park.

You will assist with daily living activities, mobility, and medication reminders, while maintaining safety and independence. The role may require local travel and using a personal vehicle. Sign-on bonus and first-day benefits reinforce our commitment to a welcoming team.

Qualifications

• Must be an active CNA in the state where PACE Organization is located.
• Minimum of one year of experience with frail or elderly population preferred.
• Valid driver’s license.
• Excellent written and verbal communication skills.
• Customer focused with ability to adapt to change.
• Proficiency with MS Office and basic email/calendaring.
• May require 10% local travel and use of personal vehicle.
• Physical ability to kneel, crouch, reach, lift, sit, walk, bend and lift.
• Ability to push/pull/move/lift a minimum of 20 pounds.

Responsibilities

• Provide personal care assistance to maintain participants' independence with activities of daily living (toileting, bathing, eating, grooming).
• Assist with mobility, exercise, and walking activities.
• Offer behavioral supervision and reminders for nursing guidance, medications, and diet restrictions.
• Engage in conversations, recreational activities, meal service, and light housekeeping.

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period. This position will be housed primarily at the LIFE Kinder Park facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred.
• Valid driver's license.
• Superior written/verbal communication skills.
• Must be customer focused, exercise independent judgement and flexibility to adapt to ongoing change.
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems.
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.
• Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
