Job summary

Trinity Health PACE in Feasterville-Trevose, PA, is seeking a Part-time Per Diem Personal Care Assistant/Home Health Aide to support participants at LIFE St Mary. You will deliver daily living assistance and monitor health status, primarily at the LIFE St Mary facility with occasional home care as needed.

The role requires an active CNA in the state, at least one year of experience with frail or elderly populations, a valid driver's license, and strong communication skills.

• Active CNA certification in the state where PACE Organization is located.
• Minimum of one year of experience with frail/elderly population (preferred).

Responsibilities

• Provide personal care assistance maintaining independence in daily living activities.

• Provide behavioral supervision or support and remind about nursing and medication instructions.
• Assist with conversations, recreation, meals, and light housekeeping.

Status: Per Diem

Location: LIFE St Mary, 2500 Northgate Dr, Feasterville-Trevose, PA

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position will be housed primarily at the LIFE St Mary facility. However, occasional home care work may be required.

What you will do

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming

• Assist participants in the maintenance of physical activity, exercise, and walking

• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance

• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need

• Must be an active certified nurse assistant in the state where PACE Organization is located.

• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred

• Valid driver's license

• Superior written/verbal communication skills

• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change

• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems

• Position may require 10% local travel and use of personal vehicle.

• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
