Job summary

SYNERGY HomeCare Of East King County is seeking a compassionate Certified Home Care Aide to join our devoted team. In this role, you will assist clients at home with daily living activities and provide companionship to improve their quality of life.

You will work both part-time and full-time shifts, delivering personalized care plans, coordinating with nurses and other professionals, maintaining accurate records, and communicating with clients and families to ensure consistent, high-quality care

Qualifications

• Certified Home Care Aide
• Provides ADL support including bathing, dressing, grooming
• Maintains client records and documents care
• Delivers companionship and emotional support
• Collaborates with nurses and healthcare professionals to plan care

Responsibilities

• Assist with activities of daily living (ADLs) including bathing, dressing, grooming, toileting
• Provide companionship and emotional support to clients
• Monitor changes in clients' condition and report to the nurse or other healthcare professionals
• Assist with light housekeeping, laundry, and meal preparation as needed
• Maintain accurate and detailed records of client care and services provided
• Develop and implement personalized care plans in collaboration with clients and other healthcare professionals
• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
