Job summary

Trinity Health PACE in the United States is seeking a Personal Care Worker to provide direct personal care to participants at LIFE Kinder Park, with some home care as needed. The role includes assisting with activities of daily living, mobility, and supervision to ensure safe, dignified care for elderly individuals.

Qualified candidates will be an active CNA with at least 1 year of frail elderly care experience, strong communication, and flexibility to adapt to changing needs.

Qualifications

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver’s license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.

Responsibilities

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

Incentives: $2500 Sign on Bonus AND 1st Day Benefits

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period.

This position will be housed primarily at the LIFE Kinder Park facility. However, occasional home care work may be required.
