Responsibilities

  • Ensure all children with special health care needs and families are treated with dignity and respect.
  • Support children and families to develop a positive self-image.
  • Assist families to participate in home and community activities meaningfully.
  • Maintain complete contemporaneous records (time sheets, documentation, objective data).
  • Attend team meetings and implement plan directives as assigned.
  • Join monthly supervisory meetings and semi-annual planning meetings.

Essential Duties and Responsibilities
  • Ensure that all children with special health care needs and families supported are treated with dignity and respect.
  • Support children with special health care needs and their families in developing a positive self-image.
  • Treat each individual within the family as a person who has unique needs and wants.
  • Support children with special health care needs and their families to function to their fullest potential.
  • Support children with special health care needs and their families to participate in their home and community in ways that are meaningful to them.
  • Encourage children with special health care needs and their families to maintain positive relationships.
  • Work with the HBTS support team in developing consistent and integrated support techniques, methods, and programs.
  • Attend and actively participate in team meetings.
  • Support and carry out all team decisions consistently and appropriately.
  • Attend in-service trainings as determined by the Program Director.
  • Gather and submit data related to achieving HBTS goals in a timely manner.
  • Home‑Based Workers are responsible to maintain the following complete and legible contemporaneous records:
    • Bi‑weekly time sheets signed daily by the parent and staff.
    • Daily documentation sheets that specify date, time, location, duration and levels of participation of parent and child for each therapeutic objective.
    • Individual scoring sheets for each objective listed in the child’s treatment plan; data compiled daily and documents the child’s response to treatment, the level of assistance provided by staff or parents, and all other objective information identified by the therapeutic consultant as relevant to the evaluation of care for each objective.
    • Bi‑weekly case presentation sheets that prepare staff for a supervisory meeting with clinical staff; case presentations provide staff an opportunity to summarize data and other relevant information for each goal. These documents are also used to record feedback and recommendations provided by clinical staff.
    • Staff meeting once a month – mandatory.
    • Meet with your clinical supervisor once a month.
  • Complete preparation materials and participate in the semi-annual planning meetings, if requested (HBTS status update and renewal, IEP).
  • Ensure the optimal health and safety of children with special health care needs receiving supports.
  • Adhere to all safety policies, including safe driving and seatbelt policies.
  • All other duties as assigned and/or as identified in the HBTS treatment plan and individual profile.
Qualifications
  • High School Diploma or GED required and at least 2 years’ experience working with children with special health care needs, or a combination of experience, education, and training working with children with special health care needs and their families, or an Associate’s Degree in a related field.
  • Proficiency using digital devices and operating systems.
  • Willingness and ability to complete and perform CPR and Non‑Violent Crisis Intervention (NCI).
  • Willingness and ability to complete HBTS Training Curriculum.
  • Valid Driver’s License and acceptable driving record required.
  • Access to and provision of an insured automobile on an ongoing basis.
  • Ability to lift independently up to 50 lbs. and up to 90 lbs. with assistance.
  • Stand for 70% of the workday, with frequent kneeling, bending, and stooping.
  • Physical capability to assist students with mobility challenges, including pushing and pulling.
