Job summary

Mosaic in Topeka, KS is seeking a Mosaic at Home Coordinator to empower individuals with intellectual and developmental disabilities to live fulfilling lives. You will build relationships with the people we serve and their families, and collaborate with the team to ensure personalized support.

This role requires travel within Manhattan and Topeka, a Monday–Friday schedule, and on‑call crisis support as needed.

Qualifications

• Bachelor’s degree preferred.
• Minimum of two years of related work experience required.
• Experience working with people with intellectual disabilities is a plus.
• Valid driver's license is required.
• Strong communication and relationship-building skills.
• Experience in behavior management and knowledge of regulations.
• Excellent planning and problem-solving skills.
• Computer proficiency required.

Responsibilities

• Build meaningful connections with individuals receiving support, families/guardians, home providers, direct care workers, and stakeholders.
• Help individuals express goals and needs, ensuring their voices are heard.
• Monitor relationships between individuals and home providers or direct care workers for positive matches.
• Collaborate to develop Individual Support Plans reflecting unique wishes and needs while adhering to policy.
• Oversee contracted services and ensure contractual compliance during visits in homes or service environments.
• Provide on-call support to Mosaic at Home Providers and Direct Care Workers as needed.
