Job summary

U.S. Department of Veterans Affairs seeks a Social Worker for the Home-Based Primary Care (HBPC) program. The role delivers psychosocial assessments, case management, and counseling to Veterans and their families in the home setting.

Responsibilities include care plan development, coordination with VA and community services, and participation in an interdisciplinary team. Requires MSW and state licensure with knowledge of medical and mental health terminology.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Licensed or certified to independently practice social work at the master's level.
• U.S. citizenship requirement; compliance with state licensure prerequisites.

Responsibilities

• Deliver high-quality psychosocial assessments and develop care plans.
• Provide individual, group, and family counseling.
• Coordinate with VA and community services for linkage to needed resources.
• Offer crisis intervention and emergency planning when appropriate.
• Supervise social work trainees or students as applicable.
• Maintain therapeutic relationships with Veterans and their families.
