Job summary

• Provide case management and clinical treatment for Veterans in HBPC.
• Conduct psychosocial assessments and counseling with Veterans and families.
• Advocate for Veteran needs and coordinate linkage with VA and community services.
• Perform caregiver burden assessments and implement interventions; provide casework services as needed.
