Job summary

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Licensure/certification required to independently practice social work.
• Proficient in English, both written and spoken.

Responsibilities

• Provide initial and ongoing psychosocial assessments of patients and caregivers.
• Actively participate in Home Based Primary Care interdisciplinary team meetings.
• Share observations to enhance quality of life for patients in the home setting.
