Job summary

U.S. Department of Veterans Affairs is seeking a Home-Based Primary Care (HBPC) Social Worker in Dayton to join a multidisciplinary team delivering intensive in-home psychosocial care for Veterans with complex medical and behavioral health needs.

You will provide case management, caregiver support, and collaborate with community partners to tailor services. The role requires a MSW from an accredited program, state licensure, and relevant clinical experience.

Qualifications

• Master's degree in social work from CSWE-accredited program.
• Current state licensure to independently practice social work.
• One year post-MSW experience in health care or related field.
• Doctoral degree may not substitute for MSW

Responsibilities

• Performs comprehensive initial and follow-up psychosocial assessments.
• Develops psychosocial care plans with interdisciplinary teams.
• Provides crisis intervention and suicide prevention in home environments.
• Coordinates care and referrals to VA and community resources.
• Delivers evidence-based individual, family, and group psychotherapy
