Job summary

Capital Health seeks a registered nurse to perform home visits for LIFE program participants, delivering high-quality care through the nursing process and collaboration with the care team. You will conduct assessments, triage, medication administration, wound care, patient education, and coordinate DME and supplies.

Mileage reimbursement is provided for distances driven. Required: Associate's degree in nursing (BSN preferred) and NJ or multi-state licensure; 40 hours per week;

Qualifications

• Associate degree in nursing required; BSN preferred.
• NJ or multi-state RN license required.
• Experience with home health and elderly population is desirable.

Responsibilities

• Perform home visits for LIFE program participants.
• Conduct nursing assessments, triage, medication administration, wound care, and patient education.
• Coordinate DME and medical supplies for participants.
• Collaborate with IDT and care team to plan and adjust care.
