Job summary

St. Peter's Health Partners in Troy, NY is seeking a Full-Time Physical Therapy Assistant for Homecare.

You will Collaborate with a team of Physical Therapists to manage a home-based caseload, focusing on post-acute or subacute patients. The position offers comprehensive orientation, mentorship, and access to modern technology, with mileage reimbursement and strong management support to help you deliver compassionate care in patients' homes.

Qualifications

• NYS PTA Licensure is required.
• Minimum one year of experience with adult/geriatric population as a PTA preferred in home care.

Responsibilities

• Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
• Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
• Care for one patient at a time.
• Collaborate with a team of Physical Therapists to manage a caseload of patients requiring home-based therapy, primarily post-acute or subacute.
