Trinity Health is seeking a Part-Time Physical Therapy Assistant to join Eddy Visiting Nurse & Rehab Association (EVNRA) for home-based therapy. You will collaborate with a team of PTs to manage a post-acute caseload, starting and ending your day from patients’ homes with orientation and mentorship.

The role emphasizes patient-centered care, independence after training, and safe practice with mileage reimbursement and modern tech tools.

Qualifications

• NYS PTA Licensure
• Minimum one year of experience working with adult/geriatric PTA
• Home care experience preferred, but not required
• Care for one patient at a time

• Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
• Evaluates and treats individuals with functional deficits due to neurologic, orthopedic and medical conditions.

Pay Range: $29.53 - $38.22 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
