St. Luke's Health Network, Inc. is seeking a Registered Nurse ACT to join a multidisciplinary Assertive Community Treatment Team. The RN ACT delivers nursing care to individuals in their homes or the community, helping manage symptoms and coordinate medical and psychiatric care.

The role includes outreach, assessment, service planning, crisis on-call rotation, documentation, and cross-training with ACT specialties to promote housing stability and access to community resources.

Qualifications

• Registered Nurse with current license to practice in the Commonwealth of Pennsylvania.
• Experience in psychiatric nursing specialty.

Responsibilities

• Meet ACT participants in homes/community to improve quality of life.
• Provide medication services and observe ingestion as directed.
• Develop, implement and revise treatment goals with the ACT team.
• Coordinate with medical providers to ensure timely health care.
