Job summary

St. Luke's University Health Network is seeking a Registered Nurse ACT to deliver in‑home and community nursing services as part of an Assertive Community Treatment Team. You will assist with symptom management, coordinate medical and psychiatric care, and participate in service planning and family consultation.

The role emphasizes outreach, collaboration with the ACT team, and ongoing documentation to ensure quality care and stability for participants.

Qualifications

• PA nursing license active in Pennsylvania
• Experience in psychiatric nursing preferred
• BLS and CPR certifications are required

Responsibilities

• Meet ACT participants in their homes or community venues to provide in vivo services
• Provide medication service including delivery and observation as directed by clinicians
• Coordinate with ACT team to develop and revise treatment goals and provide counseling as needed
• Provide on‑call crisis services on a rotating schedule as directed by team leadership
