• Plans and delivers technically competent home-based care for Veterans with complex needs.
• Utilizes interventions to maximize health and reduce unnecessary urgent care.
• Performs home visits to Veterans and delegates care as appropriate.
• Adheres to safety, quality standards and professional guidelines.
