Job summary

Florence VA Clinic is seeking an experienced Home Based Primary Care RN to provide direct Veteran care and care coordination within the HBPC program. The nurse will plan and deliver technically competent home-based care for Veterans with complex needs, using interventions to maximize health while reducing urgent care visits.

Home visits and safe delegation of tasks are essential. The role requires an active RN license, English proficiency, and completion of an accredited nursing program

Qualifications

English proficiency required for direct patient care and documentation.
Education: ADN/diploma or BSN; BSN with experience preferred; MSN may substitute for experience.
Foreign educated nurses require degree equivalency validation and state licensure.

Responsibilities

Provides direct Veteran care and care coordination within the HBPC program.
Plans and delivers competent home-based nursing care for Veterans with complex needs.
Uses interventions to maximize health and reduce unnecessary urgent care visits.
Conducts home visits to Veterans enrolled in HBPC.
Delegates elements of care to appropriate staff based on assessed needs and safety.
