Job summary

Mass General Brigham Community Physicians, Inc. in the Greater Boston area is seeking an MD or DO with Internal Medicine or Family Medicine training to join the Compass PLUS program as a Primary Care Physician.

You will care for a focused panel of about 100 medically and socially complex patients in homes and community settings with a multidisciplinary team, offering time, flexibility, and autonomy. This role emphasizes relationship-based care, health equity, and collaboration across the system

Qualifications

• MD or DO with training in Internal Medicine or Family Medicine.
• Outpatient experience in high-need settings preferred.
• Strong interest in population health and health equity.

Responsibilities

• Provide comprehensive, relationship-based primary care to a small panel of high-acuity patients.
• Partner with a multidisciplinary team to deliver care in patients’ homes and community settings.
• Develop individualized care plans that integrate medical, behavioral health, and social needs.
• Consult with primary care physicians and care teams across the system on complex patient management.
