All participants receive clinical and support from a multi-disciplinary care team.The Compass PLUS Primary Care Physician (PCP) is responsible for the care of a small panel of highly complex patients in the Compass PLUS program. Designed for physicians who thrive in high-need outpatient settings, the program combines relationship-based care, clinical flexibility, and a highly collaborative approach centered on each patient’s priorities.Rather than relying solely on traditional office visits, the Compass PLUS team meets patients where they are—primarily in their homes and community settings. The physician works alongside a Nurse Practitioner, Nurse, Behavioral Health Clinician, and Community Health Worker to address medical, behavioral, and social barriers to care.Most patients remain connected to their existing primary care physician, with Compass PLUS providing added support. For patients with the most intensive needs, the Compass PLUS physician becomes the primary provider, with the goal of stabilizing care and transitioning patients back to traditional primary care when appropriate.About the RoleThis is a unique opportunity to practice primary care with more time, flexibility, and support. You will manage a focused panel of approximately 100 medically and socially complex patients, serve as PCP for those fully transitioned into Compass PLUS, and consult with clinicians across the system on high-risk patient care. Compensation is prorated for part-time roles.
