Qualifications

• MD or DO with training in Internal Medicine or Family Medicine.
• Board certified or board eligible.
• Eligibility for Massachusetts medical licensure and DEA registration.
• Outpatient experience in high-need settings is preferred.

Responsibilities

• Provide comprehensive, outpatient primary care to a high-need patient panel.
• Collaborate with a multidisciplinary team to manage medical, behavioral health and social needs.
• Develop individualized care plans and coordinate transitions across care settings.
• Engage in population health initiatives and care coordination across the system.
• Deliver care primarily in diverse community and home-based settings.
