Job summary

Mass General Brigham is seeking a Compass PLUS physician to provide primary care for a small, highly complex patient panel in homes and community settings around Greater Boston.

You will partner with nurses and social workers, focusing on relationship-based care, population health, and reducing hospital use, with no after-hours call. This role emphasizes autonomy within a supportive team and enables transition back to traditional primary care as patients stabilize.

Qualifications

• MD or DO in Internal Medicine or Family Medicine required.
• Board certification or eligibility preferred.
• Experience in outpatient settings serving high-need populations.
• Interest in population health and health equity.
• Proven ability to work in collaborative care teams.
• Eligible for Massachusetts medical license and DEA.

Responsibilities

• Deliver comprehensive outpatient primary care to a small panel of high-acuity patients in home/community settings.
• Collaborate with multidisciplinary care teams across homes and communities.
• Develop and implement personalized care plans addressing medical, behavioral, and social needs.
• Consult with PCPs and care teams across the system to share expertise.
• Support transitions between inpatient, emergency, and outpatient care.
• Aim to reduce unnecessary hospital use and improve health outcomes.
