Responsibilities

• Performs nursing care and phone triage to guide patient actions.
• Follows six medication rights and administers medications based on patient age and condition.
• Assesses and analyzes data to plan patient care and updates nursing plan.
• Develops and updates an individualized plan of care including discharge planning.
• Participates in quality improvement activities and contributes to department projects.
