Job summary

Bloom Healthcare is seeking a Primary Care NP or PA to deliver relationship-based, home- and community-based primary care for homebound seniors in the St. Louis area. You’ll work with an interdisciplinary team and data-enabled systems to manage chronic and acute conditions with autonomy and support.

You’ll enjoy a manageable schedule, a strong mission, and extensive patient impact as you help patients age safely in place while collaborating with MD leadership and care teams.

Qualifications

• NP with a master’s degree and active Missouri licensure.
• Strong focus on geriatrics and medically complex populations.
• Excellent communication and home-based care skills.

Responsibilities

• Provide longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct specialty visits such as wound care and post-discharge assessments.
• Coordinate care and track outcomes using Bloom’s data ecosystem.
