Qualifications

Responsibilities

• Gathers and synthesizes data from patient records and direct observation.
• Develops treatment plans with measurable goals and implements procedures.
• Instructs patients and families in therapies to maximize home care and discharge.
• Coordinates care with physicians, nurses, and other CH team members.
• Maintains timely and complete documentation of care, attendance, and discharge.
