Job summary

Cynet Health is seeking a Physical Therapist for a 13-week Therapy assignment focused on home-based and long-term care settings. You will provide patient-centered care, perform assessments, and collaborate with a multidisciplinary team to develop and revise the plan of care.

The role requires at least one year of PT experience in long-term care or home care, a current state license in Physical Therapy, and BLS certification. Oasis experience is required.

Qualifications

• Minimum of one year experience in Long Term Care/Home Care setting as Physical Therapist.
• Must have Oasis experience.
• Current state license in Physical Therapy.
• BLS certification (AHA ONLY).

Responsibilities

• Provides case management and direct patient care within a team environment.
• Administers comprehensive physical therapy modalities in the home setting.
• Assists the physician in evaluating level of function and helps develop and revise the plan of care.
• Prepares clinical and progress notes and consults with the family and other personnel.
• Participates in in-service programs and ensures that Care Delivery Services align with agency vision, mission, and values.
• Promotes effective working relationships and works effectively as part of the team.
• Utilizes resources, supplies, and time efficiently to improve the value of services.
• Communicates positively, supportively, and respectfully with all external sources, patients, families, and staff.
• Strives for continuous improvement in all aspects of the Physical Therapist function.
• Develops and revises treatment and discharge plans in conjunction with patients, families, and the care delivery team.
