Job summary

Cynet Health is seeking a Physical Therapist to provide home health services in California. The role encompasses evaluating and treating patients in their homes, assisted living facilities, and other community settings, with a focus on orthopedic, cardiovascular, pulmonary, oncology, neurological, and geriatric conditions.

The therapist will manage 3–5 patients per day, perform Start of Care visits, and utilize electronic charting for documentation.

Qualifications

• Experience in home health and OASIS is required.
• Excellent oral and written communication skills.
• Ability to work in varied home environments.

Responsibilities

• Position involves providing physical therapy services in homes, assisted living facilities, senior apartments, and board and care homes.
• Therapist manages 3 to 5 patients per day depending on visit type.
• Two Start of Care visits are expected daily among evaluations.
• Use of a charting system is necessary for documentation purposes.
• Supervision of up to 2 PTA/COTAs is required.
• Flexibility to work in different territories.
