Job summary

Rehab Without Walls Neuro Rehabilitation in California seeks an experienced Physical Therapist to provide functional, home-based therapy that supports patients in daily activities and community life.

You will work independently with a supportive, collaborative team, performing evaluations, creating plans, and guiding patients and families through progress and discharge processes. An active PT license and BLS/CPR are required; 1 year of experience preferred.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in the state of practice.
• BLS/CPR certification in accordance with state regulations.
• At least 1 year of PT experience preferred.
• Knowledge of rehabilitation techniques for neurological injuries.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordination of care and improve outcomes.
