Job summary

All Care and Comcare Home Health in California is seeking a compassionate Physical Therapist to deliver comprehensive in-home therapy, including SOC, ROC, recertifications, and discharges, with a patient-centered approach to promote independence and comfort for clients in their own homes.

You will travel to patient homes, assess skilled needs, plan interventions, document progress, educate families and caregivers on home exercise programs, and collaborate with an interdisciplinary team to meet

Qualifications

• Active California Physical Therapy license/registration.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes.
• Flexibility regarding work locations throughout service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services per physician orders.
• Perform SOC, ROC, recertifications, and DCs.
• Assess patient needs, plan interventions, and evaluate outcomes.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
