All Care and Comcare Home Health is seeking a Field Physical Therapist to deliver high-quality, patient-centered therapy in home settings with a flexible schedule and supportive team.

The role requires CA licensure, a PT degree, and strong communication. You will perform SOC/ROC/Recert/DC, educate families, and travel across our service area to provide care.

Qualifications

• Active license in California Physical Therapy Board.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care.
• Collaborate with interdisciplinary team to set patient goals.
• Educate patients, families, and caregivers on home exercise programs.
