Job summary

All Care and Comcare Home Health in California seeks a compassionate Physical Therapist to provide skilled in-home services with a flexible schedule. You will work in patients' homes, delivering personalized therapy in a supportive team environment.

Responsibilities include SOC, ROC, recertifications, and discharge planning, along with education for patients and families. Must be licensed in CA and willing to travel throughout the service area.

Qualifications

• Active California Physical Therapy license required.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout service area.

Responsibilities

• Provide skilled physical therapy services per physician orders.
• Perform SOC (start of care), ROC (resumption of care), recertifications, and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care as needed.
• Collaborate with interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on home exercise programs.
