Job summary

All Care and ComCare Home Health is seeking a compassionate Physical Therapist to join our growing team. This role provides high-quality, individualized care in patients' homes while maintaining a flexible schedule and supportive team environment.

You will perform SOC evaluations, ROC, recertifications, and discharges and collaborate to meet patient goals. Requirements include an active California PT license, graduation from an accredited PT program, strong documentation skills, and willingness

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
