Job summary

All Care and Comcare Home Health in California is seeking a compassionate and motivated Physical Therapist to join our growing team. You will provide high-quality, individualized care in the comfort of patients' homes with a flexible schedule and supportive team environment.

Responsibilities include performing SOC, ROC, Recert, and DC evaluations, planning interventions, collaborating with the interdisciplinary team, and educating patients and families on home exercise programs.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
