Job summary

Big Minds, Tiny Hands is seeking a compassionate Physical Therapist to provide home-based Early Intervention services to children from birth to age three with developmental delays in New York. The role is fee-for-service with flexible scheduling and family-centered therapy aligned with each child’s IFSP.

Responsibilities include direct service in home/community settings, assessments, caregiver coaching, and collaboration with a multidisciplinary team while ensuring compliant documentation and NY

Qualifications

• Doctorate or Master’s degree in Physical Therapy.
• Experience with pediatric population (birth to 3 preferred).
• Strong organizational and communication skills.

Responsibilities

• Provide therapy services in the child's home or community setting.
• Implement individualized treatment plans based on IFSP goals.
• Address gross motor development, mobility, strength, balance, and coordination.
• Use play-based, routine-based, and evidence-based intervention strategies.
• Educate families and coach caregivers to support carryover of skills.
• Conduct evaluations and submit progress notes daily.
• Coordinate with service coordinators and multidisciplinary team members.
• Follow NY State Early Intervention Program guidelines and billing requirements.
• Ensure timely and accurate documentation and required forms.
