Job summary

TheraCare in Brooklyn, NY seeks a Physical Therapist to join our Early Intervention program. You will deliver PT services to infants and toddlers in home and community settings, focusing on gross motor development and family-centered care.

Responsibilities include creating IFSP-aligned plans, coaching families, documenting services, and collaborating with an interdisciplinary team. NYS license required; EI experience preferred; reliable transportation helpful.

Qualifications

• Licensed NYS Physical Therapist in good standing.
• Degree from an accredited physical therapy program.
• Experience in pediatric and home-based EI settings preferred.

Responsibilities

• Provide PT to children ages 0–3 in home and community settings.
• Develop and implement IFSP-aligned treatment plans.
• Educate families with home activity programs to support progress.
• Document services and maintain timely records per EI standards.
• Participate in IFSP meetings and collaborate with interdisciplinary teams.
• Adjust interventions based on child progress and clinical judgment.
