Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team to provide advanced home-based palliative care, helping patients achieve quality of life while managing complex medical needs. You will work autonomously with the Palliative Medical Director, conduct thorough assessments, develop individualized care plans, and coordinate with families and caregivers.

This role includes evening/weekend call coverage and adherence to Medicare/Medicaid

Qualifications

• Master of Science in Nursing from an accredited school of nursing.
• Minimum two years’ Nurse Practitioner experience.
• Specialty in Family or Adult Medicine preferred.
• Experience with Part B billing.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Collaborate with Palliative Medical Director and care team to deliver comprehensive palliative care.
• Perform in-depth patient assessments across physical, emotional, and psychosocial domains.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications and order tests as permitted by state law.
• Maintain timely clinical documentation per standards.
• Communicate with physicians and other healthcare professionals.
• Participate in evening/weekend call rotation.
• Facilitate referrals for additional care services when appropriate.
