Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to join an interdisciplinary team delivering advanced palliative care in patients' homes. You will work autonomously in collaboration with the Palliative Medical Director, guiding patients and families through treatment and coordinating with care teams.

Responsibilities include comprehensive assessments, care planning, medication management, and documentation with attention to Medicare/Medicaid regulations.

Qualifications

• MSN from an accredited school or equivalent per state law.
• Minimum two years of Nurse Practitioner experience.
• Family or Adult Medicine specialty preferred.
• Part B billing experience preferred.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.
• Perform in-depth patient assessments including physical, emotional, and psychosocial evaluations.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient needs and progress.
• Prescribe medications and order diagnostic tests as permitted by state law.
• Maintain timely, accurate clinical documentation.
• Communicate effectively with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
