Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team and provide advanced palliative care to patients in their homes. You will work autonomously in collaboration with the Palliative Medical Director, delivering exceptional care and guiding patients and families through every stage of treatment.

You will perform in-depth assessments, develop individualized care plans, and manage treatment with a focus on quality of life.

Qualifications

• MSN from an accredited program and NP certification/license as required by state law.
• Minimum two years of NP experience, with family or adult medicine preferred.
• DEA license and state licensing as applicable; ACHPN preferred.

Responsabilités

• Collaborate with the care team to deliver comprehensive palliative care in patients' homes.
• Perform in-depth patient assessments across physical, emotional, and psychosocial domains.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications and order tests per state law.
• Maintain timely, accurate documentation in line with professional standards.
• Communicate effectively with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
