Job summary

Christus Palliative Care St. Frances Cabrini, a part of LHC Group, is seeking a Palliative Care Nurse to provide direct patient and family care through palliative consultations for seriously ill patients not eligible for hospice.

The role requires RN/APRN licensure, prescriptive authority, and BCLS, with 5+ years of clinical nursing experience. ACHPN certification is preferred. Salary ranges from $90,650 to $135,976 annually, plus a comprehensive benefits package.

Qualifications

• RN or APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• 5+ years of clinical nursing experience.

Responsibilities

• Provides direct patient and family care through palliative care consultations.
• Ensures specialty services for patients with serious illness with prognosis ≤1 year.
• Demonstrates evidence-based palliative care practices in patient care.
• Gathers information from medical records, family, and caregivers for assessments.
• Completes comprehensive assessments and formulates plans of care.
