Resilient Healthcare Corp in Southeast Texas seeks a motivated Outpatient Physical Therapist to deliver home-based orthopedic care. You will work one-on-one with patients in their homes, collaborating with surgeons and care managers to ensure seamless care from surgery through recovery.

The role offers flexible scheduling, competitive pay, and mentorship if you are a new grad. You will travel within the service area, using evidence-based methods and documenting care under outpatient standards.

Qualifications

• Current and valid Physical Therapist license in TX.
• Experience in outpatient orthopedic or home health preferred.
• Strong interpersonal and communication skills.
• Self‑directed with ability to work independently in the field.

Responsibilities

• Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes.
• Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families about treatment options, exercises, and preventive measures.
• Conduct comprehensive evaluations and implement tailored treatment plans.
• Document all care in alignment with outpatient standards and regulatory requirements.
• Participate in ongoing case reviews and care team meetings.
• Work with and supervise PTAs.
