Job summary

Resilient Healthcare is seeking a motivated Outpatient Physical Therapist to deliver home-based orthopedic care. The role partners with orthopedic surgeons and a management team to create a streamlined continuum from surgery through recovery.

You will provide evidence-based therapy in patients' homes, collaborate with care teams, and tailor plans while educating families. Travel within the service area is required. Training is available for new grads, with a supportive mentorship environment.

Qualifications

• Current and valid Physical Therapist license in TX.
• Minimum 1 year of outpatient orthopedic or home health experience preferred but not required.
• If new grad, training and mentorship provided.
• Strong interpersonal and communication skills.
• Self-directed, with the ability to work independently in the field.
• Reliable transportation and willingness to travel within service area.
• Excellent communication skills for interacting with patients, families, and interdisciplinary teams.

Responsibilities

• Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes.
• Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Educate patients and their families about treatment options, exercises, and preventive measures.
• Conduct comprehensive evaluations and implement tailored treatment plans.
• Document all care in alignment with outpatient standards and regulatory requirements.
• Participate in ongoing case reviews and care team meetings.
• Work with and supervise PTAs.
