Penn Medicine is seeking an Occupational Therapist to support our patient-centered care model within an interdisciplinary team. You will evaluate and treat patients to maintain, improve, or restore function in ADL/IADL, and coordinate with physicians and care teams to plan interventions.

Responsibilities include documenting exams, scheduling revisits, and ensuring safe, effective therapy across home care services.

Competenze

Bachelor's Degree in Occupational Therapy from ACOTE-accredited school.
Pennsylvania OTR/L license, BLS/CPR through AHA.
Valid Driver's License.
Car Insurance.

Mansioni

Collaborates with the interdisciplinary team, schedulers, and managers to provide optimal patient experience and meet patient needs.
Performs and documents an initial exam, evaluating data to identify functional impairments; schedules revisits and reassessments.
Reviews physician's referral and patient records to determine diagnosis and required OT treatment.
Administers ADL/IADL retraining, therapeutic exercises, and cognitive retraining to improve function and quality of life.
Instructs patient, family, and caregivers in treatment procedures and home modifications to improve carry-through.
Performs medicine reconciliation within a multi-disciplinary team and monitors vital signs during sessions.
Discharges patients when goals are attained and ensures follow-up care.
Assigns patients to OT assistant and provides supervision as appropriate.
Adheres to Penn Medicine values, policies and procedures.
