Job summary

Penn Medicine is seeking an Occupational Therapist to join our rehabilitation services in a patient-centered environment. You will collaborate with a multidisciplinary team to design, implement, and monitor individualized OT treatment plans to improve ADL/IADL performance and overall quality of life for home care patients.

You will assess, document, and adjust interventions, educate patients and families, and coordinate with physicians and other providers to ensure continuity of care and safety.

Qualifications

• Bachelor's Degree in Occupational Therapy from ACOTE accredited school of Occupational Therapy - Master's or Doctorate preferred.
• Pennsylvania OTR/L license
• BLS/CPR through AHA
• Valid Driver's License
• Car Insurance

Responsibilities

• Collaborates with interdisciplinary team to provide optimal patient experience and plan OT treatment.
• Performs and documents initial exam and schedules revisits and reassessments.
• Reviews physician's referral and patient's medical records to determine diagnosis and OT treatment requirements.
• Administers ADL/IADL retraining, therapeutic exercises, and cognitive retraining to improve function.
• Instructs patient and caregivers in treatment procedures to increase carry through with the plan of care.
• Performs medicine reconciliation and monitors vital signs during treatment sessions.
• Discharges patient from occupational therapy with follow-up care or referrals.
• Assigns patients to an occupational therapy assistant and provides supervision as appropriate.
